We are seeking a dedicated Clinical Case Manager to provide a wide range of specialized case management services for individuals and families facing diverse challenges. In this role, you will work closely with a variety of populations, including children, adults, families, immigrants, refugees, and individuals dealing with mental health, substance abuse, or chronic health conditions. Your responsibilities will include offering counseling, crisis intervention, and advocacy, while maintaining a high degree of independence and professionalism. Key Responsibilities:
Meet regularly with assigned cases and stakeholders to assess needs and create personalized support plans.
Offer crisis intervention, counseling, and problem-solving to program participants in areas such as mental health, housing, and employment.
Address psychosocial challenges and offer guidance to improve engagement and self-sufficiency.
Develop, implement, and monitor individualized service plans in collaboration with participants.
Screen and advocate for participants to receive benefits such as income support and entitlement programs.
Oversee treatment plans and ensure goals are met within set timelines.
Provide financial assistance and budget management support where applicable.
Complete case documentation and reports in a timely, accurate manner.
Maintain work volume and utilize strengths-based clinical techniques to support participants' needs.
Qualifications & Experience:
Education: A Bachelor's degree in Social Work, Psychology, or a related field, with three years of experience in case management or counseling, or a Master's degree in a related field with one year of experience.
Licensure: Must hold a valid Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) certification.
Skills:
Strong communication and relationship-building skills.
Ability to work effectively with diverse teams and stakeholders.
Experience in handling confidential information with professionalism and discretion.
Proficiency in case management software and Microsoft Office Suite.
Strong problem-solving skills and the ability to adapt to new situations.
Work Environment:
This position requires an on-site presence in the Chicago area, with possible flexibility for occasional work-from-home arrangements when appropriate.
The work environment is typically moderate in noise level, and the role may require traveling to different program sites across the region.
Physical Demands:
Regularly required to talk, hear, sit, stand, and walk.
Occasional lifting of up to 25 pounds.
Ability to perform physical restraint when necessary.
